New Patients

Welcome to Lafayette Heart and Vascular Clinic! We're looking forward to caring for you. Here's what you need to know for your first visit.

What to Bring

  • Valid photo ID (driver's license or state ID)
  • Insurance card(s) - both primary and secondary
  • List of current medications with dosages
  • List of medication allergies
  • Medical records from previous cardiologists or primary care physician
  • Recent test results (EKG, echocardiogram, stress test, etc.)
  • List of current symptoms or concerns
  • Family history of heart disease

Before Your Appointment

  • Verify your insurance coverage and referral requirements
  • Request medical records be sent to our office
  • Write down any questions you have for the doctor
  • Arrive 15 minutes early to complete paperwork
  • Wear comfortable clothing
  • Bring a family member or friend if desired

Insurance & Billing

Accepted Insurance

Medicare

We accept Original Medicare (Parts A & B). No referral needed for Medicare patients.

Medicaid of Louisiana

We accept Medicaid of Louisiana (traditional Medicaid only, not managed care plans on self-pay basis).

Self-Pay

For patients with other insurance plans, we offer self-pay options. Contact our office for current pricing.

Office Policies

Appointments

  • Please arrive 15 minutes early for your first visit
  • Arrive 10 minutes early for follow-up appointments
  • We make every effort to see you on time
  • Emergency situations may cause delays

Cancellations

  • Please provide 24-hour notice for cancellations
  • Call (337) 516-2800 to cancel or reschedule
  • Late cancellations may affect future scheduling
  • We understand emergencies happen

Test Results

  • Routine results available within 3-5 business days
  • Critical results are called immediately
  • Request results through our office
  • Discuss results during follow-up appointment

Prescriptions

  • Refills require 48-hour notice
  • Contact your pharmacy for refill requests
  • Some medications require office visit
  • Keep track of when refills are due
